Admissions Team Info
Our team typically reviews and responds within 1 - 2 business days of receiving the referral paperwork from the case manager/referral agency.

Pre-Admission Meeting
Based on the assessed level of services, our team schedules a face2face meeting to ensure everyone makes an informed decision to move in to Median Community and Care.

Move In
Once a consensus has been reached, our admission team schedules the move in date and a face-to-face admission assessment.
